We offer a number of treatments and services to support you in your pursuit of your artistic work. If you are a new patient or it has been more than one year since you accessed care with us, please complete the self-referral form to share some information about yourself, your artistic practice, and your current health needs. This form should take approximately 10 minutes to complete.
This information will be kept confidential and reviewed by our Clinical Intake Coordinator to help us best direct and support you.
